Service Coordinator Supported Housing

This is a Permanent, Salaried vacancy that will close in {x} days at {xx:xx} BST.

As a Service Coordinator you’ll support vulnerable young adults, single parents and families, who may also have complex needs, living in our South of Cheshire East borough supported accommodation.

We’ll be interviewing as we go so might close the application process early if we find the right person.

Job requirements

  • Work with customers to identify their support needs and address any wider issues associated with homelessness. You’ll provide support either in their current home or help them to secure a home of their own.
  • Empower customers to regain independence and help them understand what it takes to manage their own tenancy.
  • Build and maintain positive relationships with other Concrete services and external partners to increase the level of support available to customers. This includes attending regular internal and multiagency meetings.
  • Make sure all information is recorded, collated, updated and saved securely to meet contract, service and organisation requirements and standards.
  • Work closely with the Supported Housing Team Leader to promote Concrete’s services, build positive relationships with the local community and help monitor and evaluate the service.

What we’re looking for

  • Someone educated to GCSE Level or with a good level of maths and English.
  • Great communication, problem-solving, organisational and time-management skills.
  • Someone with a genuine understanding and compassion for people without a home, who won’t judge but instead will empower customers and give them a voice.
  • Someone who is flexible and adaptable.
  • Someone with a full UK driving license who is able to use their own vehicle for work purposes.
  • DBS check.
